Evidence & Scene Management

Evidence starts disappearing the moment the incident ends.

CCTV overwrites, parts get repaired, positions get tidied and memories fade. We tell you on the first call what to protect, then collect people, parts, positions and paper systematically, with a chain of custody that holds up later.

Watch the evidence disappear

A forklift struck a worker in a warehouse. How long do you wait before calling?

Move through time and see what is still available, what is at risk and what is already gone. Illustrative example.

PEOPLE · AVAILABLE

First-hand accounts from the driver and two witnesses

Memories blend with what others say within a day or two.

PEOPLE · AVAILABLE

Details only the injured worker saw

Hospital, medication and time off make this hard to recover.

PARTS · AVAILABLE

Forklift exactly as it was, keys and settings untouched

The truck is usually back in use by the next shift.

PARTS · AVAILABLE

Brake and mast condition

Repairs start as soon as the truck is needed.

POSITION · AVAILABLE

Load, pallet and truck positions on the floor

Aisles are cleared for work, often within hours.

PAPER · AVAILABLE

Warehouse CCTV

Many systems overwrite within 3 to 7 days.

PAPER · AVAILABLE

Pre-use check sheet and shift roster

Paper records go missing or are rewritten.

PAPER · AVAILABLE

Truck telematics and impact data

Rolling memory eventually overwrites the event.

After 1 hour: 8 available, 0 at risk, 0 gone.

Call us within the first hour
Evidence plan listing people, parts, position and paper
Four Ps, each with an owner and a deadline.

Why the first hour matters most

You cannot investigate what has already been cleaned up.

1

Most evidence is lost not by design, but because people tidy up, repair and restart as fast as they can.

2

A short list of what to protect, given on the first call, saves more evidence than any expert arriving days later.

3

A proper chain of custody means findings stand up with insurers, regulators and, if needed, in court.

Use it for

The first hours after any incident

Events where insurers or regulators will be involved

Sites with CCTV, trackers or control systems

On its own it cannot

Recover evidence that has already been lost

Explain causes on its own

So we pair it with

The P3i approach

How we investigate with evidence that lasts. Fact-finding, not fault-finding.

Every P3i investigation is led by a senior investigator and supported by internationally qualified process specialists. You will not receive a list of people to blame, or a long list of generic actions. You will receive the facts, the proven causes, and a short plan that closes the gaps, in this case and across your system.

Independent

No products, repairs or contracts to sell you afterwards.

Senior-led

Thirty years of investigation, from the first call to the final briefing.

Beyond the event

Process specialists find the other cracks before they fail.

Ready to present

Written for boards, buyers, insurers and regulators.

How P3i does it for you

  1. 01

    Secure the scene

    Before clean-up, repair or restart.

  2. 02

    Collect the four Ps

    People, parts, position, paper.

  3. 03

    Save the data

CCTV, trackers, control systems, before they overwrite.

  4. 04

    Keep a chain of custody

    So findings stand up later.

  5. 05

    Index everything

So every conclusion points to its source.

Questions

What site managers ask about evidence.

Can we restart work before you arrive?

Often yes, once the scene is photographed, parts are kept and data is saved. We tell you on the first call what must be done first.

Do you do laboratory testing yourselves?

No. We carry out disciplined evidence collection at the scene and from records. Where laboratory or technical testing is needed, we coordinate with accredited providers.

How long does CCTV last?

Many systems overwrite within three to seven days. Export the relevant footage straight away.

What if the police are involved?

Official processes stay with the authorities. We work alongside them and never disturb their scene.

Protect it now. Investigate it properly.
