About P3i · Pakistan Institute of Incident Investigation

Thirty years of asking what really happened.

P3i is an independent investigation institute based in Lahore. Since the 1990s we have investigated accidents, high-potential incidents and business failures for operators, manufacturers, hospitals, insurers and project owners, in Pakistan and in 15 countries. We look for facts, not someone to blame, and we do not stop at the first answer.

Why P3i exists

Too many investigations end with a name. Ours end with a fix.

When something goes wrong, the quickest report blames the person nearest to it. The warning letter is signed, the file is closed, and a few months later the same thing happens on another shift. P3i was set up to break that cycle.

We establish what happened from evidence, find the conditions that made it possible, and then look across the organisation for the same weakness elsewhere. One investigation, done properly, closes many gaps at once. That is where the value for our clients lies.

We also train. Investigators, supervisors and leaders from more than 100 nationalities have learned the method with us, so the next investigation inside your organisation is done well too.

What we stand for

Five principles we never trade away.

01

Fact-finding, not fault-finding

We look for causes in the system of work, not a person to blame.

02

Evidence before opinion

Every finding is tied to a record, a measurement or independent accounts.

03

Independent

We sell no equipment, insurance or software, and our fee does not depend on the findings.

04

Confidential

Client names, people and case details are never published without written consent.

05

Fewer, stronger actions

A short plan with owners, dates and a check that it worked.

Who you will work with

A team built around each case.

Every investigation is led by a senior lead investigator. Around them we bring in the specialists your case needs, and you are told who is on the team before work starts.

Specialist roles are drawn from P3i's team and associates according to the case. Qualifications are confirmed in writing on request.

Today

Every kind of loss

What we do

Independent fact-finding

Incident, accident and business investigations; root cause analysis; HiPO investigation; project and multi-party reviews; support for insurers and loss adjusters; and investigation training at every level.

What we do not do

Clear boundaries

We carry out corporate fact-finding. Where specialist testing is needed we work alongside accredited laboratories; legal advice stays with your lawyers; official processes stay with the authorities.

Questions

About working with us.

Who will investigate our case?

A senior lead investigator, supported by the specialists your case needs: process, engineering, human factors, quality or clinical. You are told who is on the team before work starts.

Is P3i independent?

Yes. We sell no equipment, insurance or software, and our fee does not depend on what the investigation finds.

Do you work with laboratories and specialists?

Yes. Where a case needs specialist testing, we direct the work and coordinate with accredited laboratories, OEMs and technical specialists, then bring their results into one factual account.

Will our information stay confidential?

Yes. Client names, people and case details are never published or shared without written consent, and confidentiality terms are agreed before any records are requested.

Thirty years in. Still asking why.
